Harbinger Hospice provides the following services to individuals in their end stage of illness. These services are based in the patient's home whether that is a personal residence, assisted living, nursing facility or other group environment. We have a team of licensed professionals that provide care covering the physical, emotional, social and spiritual aspects of the person and their loved one. Along with the Medical Director overseeing the patient's pain and symptom management other team members include an RN Case Manager, Licensed Social Worker, Spiritual Care Staff, Dietician, Therapists, Volunteers and others. The person may also keep their personal physician during this time. Harbinger Hospice is unique in that we use Licensed Practical Nurses to provide the hands-on personal care to the individual. Bereavement services are offered to the families for up to thirteen months. Please contact us for information on our support groups.
